Program Participation & Professional Disclosure

It is important to begin our relationship with a clear understanding of our work together so that we can focus on achieving the outcomes you desire. Please read this Program Participation & Professional Disclosure Agreement carefully before signing. Of course, we are happy to clarify anything or answer any questions you may have before you sign.

Group Facilitator Responsibility

Your Peri Fairy facilitator, Kristen Searcy, LCSW, is fully committed to supporting you in achieving your desired outcomes and making each session meaningful and productive. Kristen Searcy, LCSW will not offer advice in any area in which she is not qualified and will, upon request, assist you in finding a trained or licensed professional for any matter that is outside her scope of expertise.

Even though Kristen Searcy, LCSW is a licensed practicing mental health professional, her relationship with you through A Desert In Bloom Collective, LLC and the Peri Fairy group services will be strictly that of a group facilitator and participant. A participant in need of traditional therapy (any modality of therapy that must be provided by a licensed therapist), psychiatric care, medications, or diagnostic evaluation will not be able to utilize the Peri Fairy group for those purposes and will be provided with a referral to the appropriate professional. A Desert In Bloom Collective, LLC, Kristen Searcy, LCSW, and the Peri Fairy group will not provide any form of professional therapy or psychiatric care. However, Kristen Searcy, LCSW can refer you, the Peri Fairy group participant, to an appropriate external resource.

Responsibility of Participant

Each participant is solely responsible for creating, implementing, and maintaining their own physical, mental, spiritual, and emotional well-being, decisions, choices, and actions, including those that arise out of or in any way relate to or result from the group facilitation relationship. As such, you, the Peri Fairy participant, agree that A Desert In Bloom Collective, LLC, the Peri Fairy group, and Kristen Searcy, LCSW are not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any group services provided by A Desert In Bloom Collective, LLC, Kristen Searcy, LCSW, and the Peri Fairy group. The participant understands that the services provided by A Desert In Bloom Collective, LLC and Kristen Searcy, LCSW during the Peri Fairy group are not therapy and do not substitute for therapy if needed, and that services provided by A Desert In Bloom Collective, LLC and Kristen Searcy, LCSW during the Peri Fairy group do not prevent, cure, or treat any mental disorder or medical disease.

You acknowledge that services provided by A Desert In Bloom Collective, LLC, Kristen Searcy, LCSW, and the Peri Fairy group do not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that Peri Fairy group participation, whether in an individual or group setting, is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by medical, legal, or other qualified professionals. It is the participant’s exclusive responsibility to seek such independent professional guidance as needed.

If any issues arise during the term of this agreement that should be addressed by a licensed therapist or physician, you agree to immediately attend to your health by contacting the appropriate professional.

Peri Fairy Group Guidelines

No Recording

A Desert In Bloom Collective, LLC, Kristen Searcy, LCSW, and the Peri Fairy group do not permit audio, video, or other electronic recording by any participants without the express written permission of Kristen Searcy, LCSW and all other participants in the session. Any violation of this policy may result in immediate termination of the services being provided.

Discrimination and Harassment Free Policy

A Desert In Bloom Collective, LLC, Kristen Searcy, LCSW, and the Peri Fairy group do not discriminate based on race, color, national origin, creed, religion, sexual orientation, public assistance status, marital status, age, disability, or sex, including sex stereotypes and gender identity, or any other characteristic protected by law. A Desert In Bloom Collective, LLC, Kristen Searcy, LCSW, and the Peri Fairy group strictly prohibit discrimination or harassment, including sexual harassment of any staff, consultant, or program participant based on any protected characteristic. If at any point you believe this policy may have been violated, please contact Kristen Searcy, LCSW.

Crisis and Emergency Services Information

If you are experiencing an emergency, dial 911. If you are in a mental health crisis that you are unable to manage, please call the Maricopa County Crisis Hotline at (602) 222-9444.

Confidentiality

The session content and all relevant materials shared by a Peri Fairy participant will be held confidential unless the participant requests in writing to have all or portions of such content released to a specifically named person or persons. Limitations of such participant-held privilege of confidentiality exist and are itemized below:

  1. If a participant threatens or attempts to commit suicide or otherwise conducts themselves in a manner in which there is a substantial risk of incurring serious bodily harm.

  2. If a participant threatens grave bodily harm or death to another person.

  3. If the group facilitator has a reasonable suspicion that a participant or other named victim is the perpetrator, observer of, or actual victim of physical, emotional, or sexual abuse of children under the age of 18 years.

  4. Suspicions as stated above in the case of an elderly person who may be subjected to these abuses.

  5. Suspected neglect of the parties named in items #3 and #4.

  6. If a court of law issues a legitimate subpoena for information stated on the subpoena.

  7. If a participant is in therapy or being treated by order of a court of law, or if information is obtained for the purpose of rendering an expert’s report to an attorney.

Please note that A Desert In Bloom Collective, LLC and Kristen Searcy, LCSW do not keep notes on individual participants in Peri Fairy as would occur in traditional therapy.
